Camp Details

• Registration Fee: $75.00 (non-refundable)
• Camp Hours: 8:00 AM – 4:00 PM
• Days: Monday–Friday
• Ages: 5–12
• Dates: June 8th – August 14th

Camper Information

To enroll your child, we collect the following camper details: child's full name, date of birth, age, and home address.

Parent/Guardian Information

We require parent or guardian contact details including name, relationship to child, phone number, and email address. An emergency contact (name, phone, relationship) is also collected if different from the parent or guardian.

Attendance Selection

Families may choose between two attendance options:

• Full-Time – Monday through Friday, 8:00 AM – 4:00 PM
• Part-Time – Select specific days of the week

Medical & Safety Information

We collect essential health information for every camper to ensure their safety, including:

• Allergies
• Medications
• Special needs or accommodations
• Physician name and phone number

Permissions

Parents are asked to provide consent for emergency medical treatment if needed, and permission for photo and video use during camp activities.

Payment Method

We accept multiple payment methods. The $75.00 registration fee is non-refundable and due at the time of enrollment.

Signature & Agreement

A parent or guardian signature is required to confirm agreement to camp policies and terms.
